A nipple and areola tattoo is a semi-permanent method to visually recreate the nipple and surrounding area on the breast, commonly used in breast reconstruction after surgery.

HOW DOES AREOLA TATTOO AND 3D TATTOING DIFFER FROM A NIPPLE RECONSTRUCTION

Nipple reconstruction is a surgery that can be performed at a doctor’s office or a surgery room. It involves surgically rebuilding and elevating the nipple.

An areola tattoo, or one with a 3D effect (with more realistic nipple design as part of Areola tattoo), is a medical tattoo that can be done by a certified tattoo artist.

WHEN IS THE IDEAL TIME FOR THE TATTOO?

It's recommended to wait at least six months after the final breast reconstruction surgery to allow proper healing before proceeding with the tattoo.

CONSIDERATIONS BEFORE THE TATTOO PROCEDURE:

If planning nipple reconstruction, it should be completed before the tattoo session to ensure optimal results. Additionally, any desired changes to the breast, such as adjusting implant position or reducing scar visibility, should be addressed before scheduling the tattoo appointment. If only one breast underwent reconstruction, only that breast will be tattooed, with efforts made to match the natural nipple and areola as closely as possible.

  • Breastfeeding or pregnant.
  • Recent surgeries (please contact your doctor)
  • Significant heart issues (consult with your doctor first)
  • Using blood thinners (please check with your doctor if can be discontinued around the time of your lip appointment)
  • Transplanted organs.
  • Diseases/ Infections that could challenge healing.
  • Accutane (have to wait a year)
  • Hemophiliac
  • Chemotherapy (Please consult with your doctor. Usually, it has to be at least six months before going through chemotherapy)
  • Prone to scarring.
  • Epilepsy
  • Hepatitis
  • Diabetes -if not regulated
  • Cardiovascular problems.
  • Have a Pacemaker (contact your doctor)
  • Having a cold, covid, flu, viral infection, high fever, etc. around the time of your appointment.
  • Autoimmune disorders (case by case basis)
  • Laser treatment in the past 30 Days.
  • Healing problems.

2. Ensure you are not tanned during the skin tone color match session.

3. Understand that medical tattoo procedures typically require 2 sessions with a 4 to 6-week interval between each session for full skin healing.

4. Exfoliate the breast area two days before your appointment.

5. Avoid taking Aspirin or Ibuprofen two days before and after each treatment session.

6. Consult your doctor before discontinuing any medication.

7. Limit alcohol and caffeine intake two days before and after each treatment.

8. On the day of your areola and nipple tattoo treatment, it's advised to shower in the morning before your appointment. However, refrain from applying any lotions, creams, oils, or powders to the skin
from your waist up.

Things to consider before scheduling your areola tattoo treatment:

1. Assess for scarring, understanding that scar tissue may not hold pigment as effectively
or may heal with cooler colors.

Identify the type of skin being worked on, considering factors like age, scarring, or stretch marks.

2. Evaluate the condition of the skin, noting dryness, slow healing, or sensitivity.

3. Determine if the tissue has undergone radiation treatment, which can make the skin thinner and more fragile, affecting pigment acceptance and healing.

4. Consider the skin tone, recognizing that darker skin tones may heal differently after the procedure.

5. Check for any previous nipple grafts, radiation treatments, noting potential challenges in pigment retention.

6. Determine if one or both areolas are being treated, allowing for additional time and color matching considerations.

7. Note the date of the last surgery, ensuring a minimum waiting period of six months or providing a note from the surgeon.

8. Assess breast symmetry and overall health, including any medications being taken.

9. Request a doctor's note if unsure about the impact of medications on the treatment.

Upon arrival, there will be a discussion regarding:

  • Addressing any questions regarding potential discomfort during the process.
  • Any potential characteristics of the final product you may be particular about.
  • Desired placement on your body.
  • Selecting a shade/tint that complements your current skin tone for a realistic outcome.
  • Furthermore, your health history will be reviewed, and expectations for both during and post-treatment will be clarified.

Designing areolas is not a one-size-fits-all process.
We prioritize a personalized approach, beginning with creating a pre-designed digital likeness of your new areola based on photographs you provide.

This collaborative process allows for feedback before your appointment, ensuring that we can tailor your treatment to your preferences regarding color, size, and other factors.

Firstly, we'll work together to create the perfect shade/tint of tattoo ink that you're satisfied with. If you've undergone reconstruction for just one breast, we strive to match the most representative shading of your nipple as possible.

Our pigmentation process involves delicately applying soft tints of color into the skin layer by layer over several sessions, with at least a one-month gap between each visit. These pigments need to penetrate through scars, thin skin, grafts, and other irregularities, which is why achieving natural skin tones may require multiple visits. It's important
to note that exact matches are not always possible due to the natural variations in skin tones caused by factors like temperature, tanning, and circulation changes.

PMR’s commitment to hygiene is paramount, employing sterile disposable needles for each client and of course adhering to the highest standards
of cleanliness and best practices to ensure optimal results.

If a client has had nipple reconstruction, we'll tattoo both the reconstructed nipple and a rounded area

to replicate the areola. If nipple reconstruction has
not occurred in the past, we apply pigment and shading techniques to mimic the characteristics of a natural nipple. While the tattoo itself won't actually have a raised effect, it will appear to have one due to the 3D illusion.

The sensation during a nipple/areola tattoo process is typically limited to a gentle pushing and/or light shaking, especially if you've experienced a loss of sensation in your breasts after a mastectomy. Using a topical anesthetic (applied as numbing agent to skin) is not typically recommended as it can affect the pigment/shading of the finished product (unless absolutely necessary).

Your appointment is expected to last between 2.5 to 3 hours. Once the pigmentation portion of the process is complete, we'll provide you with aftercare instructions to follow. Refer to the following section for more details.

If you don’t have allergy to plasters or band aids, here are the instructions to follow

  • Keep the plaster or film on for 7 days; they are designed to be waterproof for use during showering.
  • If you notice any fluid buildup resembling a blister, remove the plaster in the shower, allow water to wash over the tattoo, gently pat dry, apply the provided cream, and then put on a new plaster.
  • Avoid exposing the tattoo to open air for more than 15 minutes.
  • On the seventh day, remove the plaster in the shower, then apply the cream both in the morning and 
at night for an additional 7 days. If the skin still feels tender, you can protect it from clothing irritation 
by covering it with cling film or Saran wrap.
  • Some redness around the skin is normal, but if it persists for more than 48 hours, please contact your doctor.
  • For the next 14 days, refrain from swimming, using saunas, or taking long showers or baths to aid in the healing process.

If you’re allergic to plasters or band-aids, here are the aftercare instructions

Day 1-7

  • Morning and night, boil water and allow it to cool. Then, use the cooled water to bathe the areola.
  • After bathing the areola, apply ointment and cover it with food wrap, cling film, or Saran-wrap. This helps prevent the areola from drying out.
  • Avoid picking or pulling at the color as it may result in patchy healing.

On day 7

  • Continue applying the cream morning and night for another 7 days.

If the skin remains slightly sore, you can use cling film or Saran-wrap over the tattoo to protect it from rubbing against your clothing. Some redness around the skin is normal, but if it persists for more than 48 hours, please contact your doctor.

For the next 14 days, refrain from swimming, using saunas, or taking long showers or baths to ensure proper healing.

AFTER THE TREATMENT, YOU MAY EXPERIENCE

  • Initially, the color intensity may appear significantly sharper and darker than desired. It can be around 30 to 50% darker and may take 4-10 days to lighten.
  • The true color will start to become visible about a month after each application. However, the color may vary depending on factors such as skin tone, type, age, and conditions. Skin types vary in their acceptance of color, so an exact effect or color cannot be guaranteed.

  • Following the procedure, there may be swelling and redness of the skin, which typically subsides within 1-4 days depending on your lifestyle. Occasionally, bruising may occur.
  • Allergic reactions to the pigment are rare but possible.
  • Slight swelling and redness may occur after each treatment session, and in very rare cases, slight bruising may be observed.

THE OUTCOME OF THE PROCEDURE CAN VARY SIGNIFICANTLY AND IS INFLUENCED BY SEVERAL FACTORS:

Medication and skin characteristics:
Factors such as medication, skin type (dry/oily/sun-damaged), and natural skin undertones play a role in the final result.

Lifestyle choices:
Alcohol intake and smoking habits can affect the healing process and the longevity of the tattoo.

Health conditions:
Certain health conditions like a compromised immune system, diabetes, and others can impact how the skin heals after the procedure.

Diet:
A poor diet can affect overall skin health and, consequently, the outcome of the tattoo procedure.

Post-procedure care:
Proper aftercare treatment following the procedure is crucial for optimal healing and results.

Skin condition:
The condition of the skin, whether damaged/mature or healthy, also affects the outcome of the procedure. Damaged or mature skin may require special attention and may yield different results compared to healthy skin.
